AUSTRALIA - In order to examine rates and predictors of referral for hep C virus HCV ; treatment and preliminary treatment outcomes in injecting drug users IDUs ; receiving opioid replacement treatment, a prospective clinical audit was undertaken in an inner city Sydney drug dependency treatment practice between December 2002 and November 2005. The majority of IDUs 178 237; 75% ; were HCV antibody positive, of whom 170 were HCV treatment nave with no absolute treatment contra-indications. Among these 170 patients, 121 71% ; had chronic HCV. Based on risk factors for HCV disease progression, 63 of 121 52% ; chronic HCV patients were targeted for referral; these patients were older, had higher alanine aminotransferase levels and longer estimated duration of HCV infection. Of these 63 patients, 43 were referred to a hepatitis treatment clinic, and 27 attended during the audit period. Patients who attended for treatment assessment were more likely to have genotype 2 or 3, but sociobehavioural factors were similar. Liver biopsy was performed in 20 patients, with moderate or greater fibrosis in 18 patients. Of 14 patients commenced on pegylated interferonalpha and ribavirin therapy, one ceased treatment due to non-response, 10 have completed treatment, all with an end-of-treatment n 4 ; or sustained virological response n 6 ; , and treatment is ongoing in three. The development of HCV treatment referral criteria has allowed prioritisation of patients for referral, potentially halving those that require early assessment. Most researchers believe it's wise for people who are co-infected with HIV and HCV to treat their HIV infection first. However, if the liver disease is severe, then it may be appropriate to treat the HCV first. Starting therapies for HIV and HCV at the same time is discouraged as this dramatically increases the likelihood of side effects. In general, at least a one to two month gap between starting anti-HIV and antiHCV therapies is encouraged.

Interferon and ribavirin treatments
Question 22: What is your recommendation for a woman has been undergoing estrogen-only therapy for many years and starts experiencing an increase in hot flashes? Answer: This actually hits a number of concerns. The fact that she's now having hot flashes may point to the hormone delivery system. Is she taking an oral estrogen? Is she absorbing that estrogen appropriately? Does her thyroid factor into this equation at all? One might consider evaluating her hormone levels. If it appears that this person has been prescribed an appropriate dose sufficient to treat her symptomatically, why isn't it working for her? Getting serum values and checking thyroid-stimulating hormone may be considered. Looking at that woman's vagina is helpful. The vagina tends to be the endpoint of the recipient of HT and if she's beginning to show atrophy and didn't have it before, then the problem may point to her inability, for whatever reason, to absorb estrogen as well as she should be. Question 23: Do you recommend leaving an early menopausal woman on HRT for 4 or 5 years? Answer: Every patient is individualized. There are a percentage of patients who will have hot flashes for life, and a small percentage who will never have them, with the average being 4 to 7 years of menopausal vasomotor symptoms. When a patient has been on HT for several years on a dose that is effective, the decision to reduce the dose or go off therapy entirely should be hers. The patient is the best bioassay; she is the judge of how she feels. At each drop in dosage, she may say, "Yes, I feel fine or no, I don't." Question 24: If a patient was on HT during perimenopause, used it successfully for several years, and then discontinued it when the WHI studies came out, can she safely be placed back on therapy, assuming no health risks are present? Answer: The bottom line is, yes, she may--and safely, assuming there are no health risks. The decision can be based on Dr. Langer's unified hypothesis: using HT in younger women at lower doses for the shortest amount of time. The hormone can be safely restarted as long as the patient is symptomatic enough to need it. That's a clinical judgment. Question 25: What are your thoughts regarding restarting therapy in a 59-yearold woman with her uterus and ovaries intact? Combination with ribavirin in late 2002 or early 2003. The sustained response rates with these two interferons in combination with ribavirin are approximately 55%. Differences between the two treatment combinations are subtle and will only be defined if there is a large head-to-head clinical trial. Emerging data show that overall response rates with pegylated interferon plus ribavirin may be in the 60% range, and nearly 50% in patients with HCV genotype 1 virus. These numbers would apply to those patients who take more than 80% of the prescribed medicines for the full time interval specified by their genotype or viral load. A phase II trial involving previously untreated hepatitis C patients without cirrhosis suggested that 180 micrograms of pegylated interferon alpha 2a is the optimal dose. The results showed a 36% sustained response rate with pegylated interferon compared to a 5% sustained response rate with interferon alpha 2a. A larger phase III trial involving 1, 219 patients compared pegylated interferon alpha 2b with standard alpha 2b. The sustained response was 24% with pegylated interferon alpha 2b compared to 12% with interferon alpha 2b.3, 4, 5 Only two trials have evaluated pegylated interferon in patients with cirrhosis and compensated liver disease. One of the trials enrolled only patients with cirrhosis. This study found a 29% sustained response rate with pegylated interferon alpha 2a compared to a 6% response rate with interferon alpha 2a. In the other international, multi-center trial, 28% of the patients had cirrhosis. This group of patients had an overall sustained response rate of 39% for pegylated interferon alpha 2a compared to a 19% sustained response rate for interferon alpha 2a.6 Early studies indicate there may be a difference in the sustained response rates between pegylated interferon alpha 2a and pegylated interferon alpha 2b. However, the data from these studies have not been analyzed by HCV genotype and viral load, which could influence study results. Further, these are not head-to-head studies. Therefore, no conclusion can be made about the differences in overall effectiveness between these two forms of pegylated interferon. Only a detailed analysis of these trials will tell us if there is a difference between the two currently available pegylated interferons. We do not yet know how effective either type of pegylated interferon will be when given with ribavi4in in the general public. See Section 3 of this chapter for the clinical trial results on this therapy. Pegylated therapies are currently being investigated. Ongoing studies will tell us if these agents have an equal or better side effect profile than other therapies. They will also tell us how the addition of rinavirin will affect the side effect profile of pegylated interferon. THERAPIES THAT MODULATE THE IMMUNE RESPONSE Vaccines Vaccine research has historically centered on trying to prevent infection. Recently, vaccine research has taken a new direction. The trend has been toward developing vaccines capable of protecting people from chronic infection, or modifying the course of a chronic infection. There are preliminary data suggesting an HCV vaccine used in chimpanzees can stimulate them to make antibodies and inflammatory CD4 + T-cell immune cell ; responses. The vaccine is made of recombinant envelope proteins, proteins on the outside of the virus. The antibody and T-cell response prevented chronic infection in the majority of the animals in this very small study.7 Recent advances in recombinant protein technology, novel adjuvants, and DNA-based vaccines will play a major role in providing new techniques for the development of HCV vaccines.

Unrecognized physical urticarias may account for approximately 20% of all cases of chronic urticaria. A major distinguishing feature of the physical urticarias is that attacks are brief, lasting only 30 to 60 minutes. In typical urticaria, individual lesions last from hours to a few days. The one exception among physical urticarias is pressure urticaria, in which swelling may last for several hours.
